Pocono Foundation Endowment Fund


Lehigh Valley Health Network - Pocono Foundation 

The grant application must be received by March 15, 2024.

Community Support: Programs that support LVHN-Pocono Foundation’s commitment to community outreach will be considered for funding. Programs must align with LVH–Pocono’s mission to offer care, comfort, and healing. This endowment was established to provide support to programs that are aligned with this mission and to direct funding where it is needed most in the community.

Policies:
  • Notification of grant awards will occur by May 3rd.
  • Funds are committed on a one-time basis, although consideration will be given to further funding requests, which are submitted in subsequent years.
  • Approval of a grant in no way implies a commitment to continue support in the future.

Recipient Requirements:
  • Use funds by March 1st of the following year.
  • Submit a final report to the Pocono Foundation Board of Directors indicating how the funds were spent, evaluation of the results, and any unforeseen problems.
  • Attend the Annual Grant & Scholarship Award Ceremony at LVH-Pocono on May 16, 2024.
  • Submit a letter of appreciation to the Pocono Foundation Board of Directors.

In addition, please send to Denise Du Bois at denise.dubois@lvhn.org: Proof of the latest IRS determination of tax-exempt status, a current operating budget for the organization, and a list of names and affiliations of officers and directors or trustees. If the organization has been classified or applied for classification as either a Section (A) (2) organization under the IRS code, it is necessary that you provide satisfactory information that the requested grant will not result in loss of your status as a publicly supported organization.
